Beverly Ann Forsythe, age 74, of Tampa, Florida, formerly of St. Charles, Illinois passed away Monday, July 31, 2017 at her home.
She was born August 12, 1942 in Chicago, Illinois to Harry and Jeannette (nee Chmielewski) Kusmer.
Mrs. Forsythe is survived by three children, Charles, James, of Tampa and Mia of California, 13 grandchildren, a brother, Ray Kusmer of California and a son-in-law, Steven Breen of St. Charles
She was preceded in death by her husband, Robert Forsythe, her daughter, Lisa
Funeral service will be held 10:00 A.M., Tuesday, August 8th at Yurs Funeral Home, 405 East Main Street, St. Charles. Burial will be at Whitney Cemetery, Campton Hills. Visitation will be Monday from 4-8:00 P.M. at the funeral home

She has a special place in all of our hearts forever. She was a terrific mom and always treated me with respect and kindness when I taught all of her children at Lily Lake Grade School in Maple Park, IL. May she rest in peace and find eternal love and contentment. "
